You can get a copy of your medical records by completing an Authorization for Release of Patient Information form. Mail your form with a $5 processing fee made payable to San Mateo Medical Center (check or money order only) to the location where services were received. Locations are listed on the back of the authorization form. Or, you can send your request to:

Requests also can be made in person at the location listed above. There is no fee for medical records sent to other health care providers.
