“Dr. Velasco is respected and loved by his patients. He has a huge SMMC patient population yet finds time to volunteer his services in the community. He is very interested in chronic disease management and was our first provider to participate in the SEED (Spreading Effective and Efficient Diabetes Care) program which focuses on improving care to patients with diabetes through the use of a chronic disease electronic management system. He facilitates weekly group visits to educate his patients on self-management action plans and as a result, his patients have improved clinic outcomes.”
