If you do not have insurance, our Community Health Advocates will help you find the best health coverage program for you. These programs include Medi-Cal, Healthy Families, Healthy Kids, and Access and Care for Everyone (ACE).

Call (650) 573-3595 to schedule and appointment to see a Community Health Advocate or come to one of our clinic locations.
 
Locations
Community Health Advocates are located at all of our clinics. Click on a location for hours and directions: Coastside Clinic, Daly City Clinic, South San Francisco Clinic, Fair Oaks Clinic, Fair Oaks Children's Clinic, Willow Clinic, or our Main Campus. Application assistance also is available at community enrollment sites. Please call the Community Health Advocates Hotline to schedule an appointment.
 
What to Bring
Call (650) 573-3595 to find out what documents you should bring with you. Some documents you may need are listed below. If you don't have any of these documents, a Community Health Advocate can help you determine other documents you can use. We are here to help you!
  • Proof of income
  • Proof of San Mateo County residency
  • Proof of identity
  • Proof of U.S. Citizenship or immigration status (if applicable)
  • Proof of assets (if applicable)
